JOB TITLE: Document Control Clerk



The Medical Records Specialist reviews and processes requests for medical records information. They use communication skills and attention to detail abilities to oversee the management of heath care data in accordance with Texas state guidelines and HIPAA regulations. The Medical Records Specialist is the custodian and caretaker of the organization’s medical records.


• Demonstrate a knowledge of HIM department functions and operations as well as maintain an understanding of other departments functionalities
• Efficiently process medical record requests according to established procedures and release records to physician offices, attorneys, patients, insurance companies, and special requests/subpoenas as received
• Accurately review medical records to ensure all charts contain correct patient information and correct account information
• Retrieve requested information from web chart, clinic facilities, or storage as needed based on authorization
• Document request status by updating patient account notes in the practice management system
• Communicate with leadership, clinical staff, and HO employees regarding problems with medical charts in order to resolve issues in a timely manner
• Meet compliance deadlines while maintaining accuracy and upholding department quality standards
• Continually safeguard a patients privacy by ensuring only authorized individuals have access to the patients’ medical information
• Reviews all authorizations confirming they are HIPAA compliant before releasing the information
• Demonstrate an understanding of state retention policies and management of the records once they have reached the storage facility.
• Check email, voice mail and answer phone calls daily
• Adhere to all HIPAA regulations; maintain strict confidentiality requirements
• Perform other duties as assigned daily to maintain HIM workflow
• Work professionally, effectively, and efficiently in a team environment with patients, leadership, clinical staff, and HO employees


• High School Diploma or equivalent
• Administrative experience in a healthcare setting or previous experience in Release of Information is preferred
• Knowledge of release of information processes in accordance with HIPAA regulations
• Knowledge of medical terminology and patient record maintenance
• Proficient in the use of the computer and Microsoft Word and Excel
• Laser Fiche
• Ability to work with a high degree of accuracy and attention to detail
• Highly organized and meticulous
• Excellent verbal and written communication skills
• Must be able to multi-task effectively
• Solid research and problem solving skills
• Operate basic office equipment (phones, faxes, copier)
• Professional image and excellent customer service skills.
